The dental plan has a maximum benefit of $3,000 per person per year.
Preventive services include two routine cleanings every 12 months, X-rays (up to 4 images) once every 12 months, and full mouth or panoramic X-rays once every 5 years.
There is no waiting period for dental services. Benefits are effective starting on Day 1 after enrollment.
Benefits are subject to deductibles, coinsurance, and maximum limits. If you visit a non-participating dentist, additional out-of-pocket costs may apply. You may need to submit a claim to receive benefits.
